untangle Cache-Control ABNF

Description

Change Cache-Control ABNF to be generic and not to enumerate the individual directives defined by the spec itself.

Instead, for each directive, note the individual microsyntax.

We also should:

  • state what to do when a directive defined to be valueless has a value (ignore the directive or the value?)
  • clarify what to do when values are using quoted-string when the definition doesn't mention it
  • state that new directives must consider the points above, and optimally allow both token and quoted-string syntax for consistency
